Do I have to take part in the survey?
No, all of our surveys are voluntary. If you do not want to take part it will not affect your healthcare in any way. If you don't want to take part, you can return the questionnaire to us blank or phone us on our Freephone Helpline 0800 013 2064.

Is the survey confidential?
Yes, the survey is confidential. For most of our surveys, we have your name and address so that we can send you the questionnaire. We will not let anyone at the hospital or GP practice know who responded to the survey or what they said in a way that could identify them. There is a barcode/serial number on the front of the questionnaire just so that we know who has responded and so that we don't send reminders out unnecessarily.

Where do I send the questionnaire to?
You should have received an envelope to send your questionnaire back in. If you can't find it, please call us and we'll send you another one. Or, you could send the questionnaire back to:
