How do I take part in a survey?

Most of our questionnaires are sent to patients at home. They normally ask a series of multiple choice questions - you simply put a tick next to the response you choose. If you make a mistake you can cross out the tick and tick another box.

For some of our surveys you can complete the questionnaire on-line. If your survey is available via the internet, then instructions including the weblink will be given in the letter sent to you with the questionnaire.
